To begin scheduling, you'll first need to select in what Mayo Clinic region you want to schedule your COVID-19 vaccination appointment. If you're using the Mayo Clinic app, once you've logged in, you'll need to tap "Request appointment" and then "REQUEST APPOINTMENT NOW" to get to the Schedule Appointment page, then select COVID-19 Vaccination. On the new appointments page, look for the "Tell us why you're coming" section, and select the box that says COVID-19 vaccination. Once you're on that page, select the New Appointments tab. You're now on the homepage of Patient Online Services. Type in your username and password on the screen. To do this, go to and select log in to Patient Account.
